Under HIPAA, healthcare providers, including clinics and medical professionals, are required to protect your sensitive medical data and to ensure its confidentiality.

This HIPAA Information Sheet is provided to inform you of your rights regarding your personal health information (PHI) and how it will be handled within our practice. It is our commitment to follow HIPAA guidelines in safeguarding your information.

Your Rights Under HIPAA:

As a patient, you have the following rights regarding your health information:

Right to Access
You have the right to request a copy of your medical records or a summary of your health information. We are required to provide this to you within 30 days of your request. A fee may apply for copying or mailing of documents.

Right to Request Corrections
If you believe that your health information is incorrect or incomplete, you have the right to request corrections (also called amendments) to your records.

Right to Confidential Communications
You have the right to request that we communicate with you in a certain way or at a certain location (e.g., by phone, mail, or email), especially if communicating by regular means could endanger your privacy.

Right to Request Restrictions
You can request restrictions on how we use and share your health information. While we are not required to agree to all requests, we will do our best to accommodate them where possible.

Right to Privacy Notice
You have the right to receive this Notice of Privacy Practices. This document outlines how we handle your personal health information and provides details on how it may be used and shared.

Right to File a Complaint
If you believe that your privacy rights have been violated, you have the right to file a complaint with our practice or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.

Healthcare Operations
Your health information may be used for routine healthcare operations such as quality assurance, patient satisfaction surveys, or training of healthcare staff.

Authorization for Disclosure
We may disclose your health information to third parties only when you have signed a written authorization form, such as when you wish for your information to be shared with family members, friends, or other providers outside of our practice.

As Required by Law
We may disclose your health information as required by law or in response to legal proceedings, subpoenas, or court orders.

Public Health and Safety
We may disclose your health information to government agencies for public health activities, such as reporting disease outbreaks or addressing issues of safety.

Research and Teaching
In some cases, we may use your health information for research or teaching purposes, but this will always be done in accordance with HIPAA rules. Identifying information will be removed to protect your privacy.
